Presently 38 wpm (...well I know that's not already real QRQ); increasing by present training.

Schurr Profi 2 dual-lever paddle

No cw keyboard used so far. Think I'll use one if I can no longer send code accurately using a paddle.
Your favorite cw sending software
Not used so far -- perhaps it would be CWType or FLDigi .. or the simple tool FLWKey (for the WinKeyer).
Your Rig, Antenna and RF Power
TS-590S,SPE Expert 1k-FA, X7 beam, up to 1 kW.

No preferences so far.
Your favorite headphones or speakers for listening to cw
Headphones with a volume regulator integrated. This allows to attenuate the basic and at a constant level present noise level at the transceiver's headphones output jack.
